Testing Window (Certification & Recertification)
Monday, December 01 — Saturday, December 06, 2008

*Certification* Application Postmark Deadlines
Regular — Friday, July 11, 2008
Late — Monday, September 29, 2008

*Recertification* Application Postmark Deadlines
Regular — Friday, August 22, 2008
Late — Monday, September 29, 2008

Applications will not be accepted with postmarks after the late deadline for your type of application. CBNC encourages candidates to mail applications with a form of delivery confirmation.

*Cancellation*
Cancellations must be in WRITING with signature of applicant. It must be received in the CBNC office by Friday, November 21, 2008 in order to be issued a refund, less the $100 non-refundable administrative fee. Candidates must first cancel with Prometric, acquire a cancellation number and then notify CBNC in writing of the cancellation number and statement to cancel (with signature). Candidates who cancel after November 21 will not receive any refund.
